Dr. Philip Ovadia has performed more than 5,000 heart surgeries across 25 years in the operating room. His conclusion after all of it: the vast majority of those surgeries never needed to happen.
In this episode we go after what really causes heart disease, and why the explanation medicine has run on for 50 years doesn’t hold up.
We start with the obvious question: is cholesterol the cause of heart disease, or has it simply been showing up at the scene of the crime? Dr. Ovadia’s fireman analogy will permanently change how you read a lipid panel. Firemen are at every fire and never where there isn’t one. That doesn’t make them arsonists. Cholesterol is the repair crew, not the cause.
The actual root is insulin resistance and the inflammation that follows it. So we get practical about how to test for insulin resistance: the fasting insulin test that costs under $20 and almost no doctor orders, the HOMA-IR you can calculate yourself in under a minute, and the LPIR test that measures the quality of your cholesterol particles instead of the quantity. He also answers what is a normal fasting insulin level, and explains why the “normal” range printed on your lab report is set far too high to protect you.
Then we get into the medications. Do statins actually work, and if cholesterol were truly the primary cause, why is the absolute risk reduction only 1 to 4 percent? We also cover what more than five years of statin use does to insulin resistance and type 2 diabetes risk. And we settle the question of should I take a baby aspirin every day, including the one specific number on a coronary calcium score that changes the answer entirely.
On food: does red meat cause heart disease? His billboard answer is “meat is medicine,” and he defends it as essential to heart health rather than a threat to it. That leads straight into my own bloodwork. After my 90-day carnivore experiment, my total cholesterol hit 450 and my LDL hit 350. So is high LDL dangerous on carnivore? Dr. Ovadia reviewed my labs and told me my heart disease risk had actually gone down. His reasoning is worth the watch on its own.
